Assumption of risk

I acknowledge that I am voluntarily participating in dog boarding with Koda's Classroom LLC. Koda's Classroom LLC will exercise all necessary and proper care and precautions to keep all participants and dogs safe during their stay in order to prevent injury to any person, property, or dog. However, because dogs are animals and may be unpredictable, Koda's Classroom LLC cannot protect against every potential injury, risk, or accident. This release and waiver are intended to shift the risk of boarding to the participant, owner, or guardian of the dog. I acknowledge that I have had full opportunity to discuss any and all concerns I have regarding the foregoing risks with Koda's Classroom LLC and its authorized representatives.


Boarding services

Dogs will be housed in Koda's Classroom LLC's home and attached facility, with access to safe, clean, and secure accommodations. Daily exercise, feeding, and enrichment will be provided as outlined by the owner's instructions.


Health requirements

All dogs must be current on vaccinations, including rabies, distemper, and parvovirus. Proof of vaccination is required before boarding.

Dogs must be free of fleas, ticks, and other parasites. Dogs arriving with signs of infestation will be treated at the owner's expense.

Owners must disclose any medical conditions, allergies, or medications required for their dog before boarding begins.


Behavioral requirements

Dogs that can be aggressive and pose a threat to other animals or people must be approved for boarding via day trial to make sure it is a safe and doable situation. Koda's Classroom LLC reserves the right to refuse or terminate boarding services for dogs displaying aggressive or dangerous behavior that was not discussed in consultation. Owners must disclose any known behavioral issues (e.g., separation anxiety, excessive barking, chewing).


Long-term boarding

For stays exceeding two weeks, weekly updates will be provided to the owner, including photos and written progress reports. Additional fees may apply for extended stays or special accommodations.


Payment policy

A 50% deposit is required at the time of booking to secure your reservation. This deposit is non-refundable if cancellation occurs within 48 hours of the boarding start date. Payment in full is required upon drop-off of your dog. Failure to pay may result in additional fees and inability to schedule future services.


Emergency medical care

In the event of a medical emergency, Koda's Classroom LLC will make every effort to contact the owner or their emergency contact. If neither can be reached, Koda's Classroom LLC is authorized to seek veterinary care at the owner's expense.


Cancellation policy

For cancellations made within 48 hours of the scheduled drop-off time, 50% of the total boarding fee will be charged as a cancellation fee.


Release of liability

Participant agrees to hold harmless Koda's Classroom LLC from any and all claims of injury, loss, cost, expenses, or damage caused by the actions of participant's dog during the course of boarding or under the care, custody, or control of Koda's Classroom LLC. I hereby accept and assume, without reservation, all risks associated with boarding my dog, including but not limited to: the risks of injury to my dog during boarding or play with other animals. The risks that my dog may cause injury to other persons, dogs, or property. The risks of illness or injury that may occur despite all reasonable precautions.


As lawful consideration for participating in the training program, I, for myself, my heirs, next of kin, executors, administrators, legal representatives, successors and assigns (the "Releasing Party"), hereby waive, release, discharge, and covenant not to sue and to indemnify, defend, and hold harmless Koda's Classroom LLC, and their members, managers, agents, and employees (the "Released Party") from any and all injuries, losses, claims and damages to any person or persons of any nature, its directors, officers, employees, agents, sponsors, organizers, volunteers, or other representatives, their successors and/or assigns (hereinafter collectively the "Releases") from any and all liability to the undersigned for any loss, harm, damage, claim, or injury or accident to the person or property of the undersigned, whether caused by any ordinary negligent act or omission of the Releases while the undersigned or his/her dog is engaged in or participating in dog training, boarding, or using any trainer or business facilities in connection therewith.


No guarantees

It is impossible to predict how any animal, including dogs, will respond in every situation. It is impossible to guarantee any animal's future health, temperament, or behaviors. It is impossible and unethical to guarantee results of training or behavior modification for any animal or person. Owner understands that there are no guarantees of specific results being given by Koda's Classroom LLC. Owner will be required to do follow-up sessions to achieve training goals. Owner acknowledges that the majority likelihood of success lies with owner's ability to be committed and consistent with the training techniques described. Owner understands that for the training to be most effective, everyone in the household should be present, attentive, and committed to the process. It is imperative that anyone outside of the home (other dog care providers, household staff, friends or family, etc.) who is in regular contact with the dog or dogs also understand and be consistent in the training methods described if maximum success is to be achieved.


Recommendations

All behavioral advice and training suggestions are provided at the owner's own risk. Koda's Classroom LLC are not veterinarians, certified canine nutritionists, or certified behaviorists. Owner will discuss any and all changes to your canine's diet, exercise regimen, supplements, medications, or lifestyle with a qualified canine professional before implementing any suggested or offered changes, additions, or alterations to your canine's lifestyle. Use of this information is voluntary and neither Koda's Classroom LLC, its employees, nor trainer is responsible for injury to any human or animal.


Photo and video release

Owner hereby grants Koda's Classroom LLC the absolute right and permission to copyright, use, re-use, publish, and republish videos, photographic portraits, or pictures of my pets taken during consultations, training sessions, and in relation to other contexts. The photos and videos may be used for advertising, marketing, educational, trade, or any other purposes whatsoever without compensation to owner.


By completing this form, I certify that I have read this document and I fully understand its content. I am aware that this is a release of liability and a contract, and I sign it voluntarily of my own free will.
